Patient safety fails in the gap between policy and practice. I close that gap.

Over 13 years in the NHS and international health systems, from ward to executive patient safety leadership, taught me the same lesson on repeat: organisations document learning and still don’t act on it. I built Patient Safety 101 and CASCADE to close that gap.

I still practise, because I don’t build systems I haven’t tested against the reality they’re meant to survive.
