CASCADE is a working patient-safety learning demonstrator currently being tested with users using synthetic data. We are developing evaluation and pilot partnerships with health and care organisations. CASCADE helps health and care organisations see what they tried before, whether it was implemented and whether it worked, before deciding what to do next.

04In my own words
Patient safety fails in the gap between policy and practice. I close that gap.
Over 13 years in the NHS and international health systems, from ward to executive patient safety leadership, taught me the same lesson on repeat: organisations document learning and still don’t act on it. I built Patient Safety 101 and CASCADE to close that gap.
I still practise, because I don’t build systems I haven’t tested against the reality they’re meant to survive.
